How Do I Get a Prescription Refill?
Call the Sports Medicine & Orthopaedic Centers office at and speak with our staff or leave a message listing the specific medication being requested, dosage, pharmacy, and pharmacy location. You may also use the ModMed electronic health record portal to make a request.
Please plan ahead – Due to the office schedule, it may take longer than 24 hours to fill a prescription request, particularly on surgical or clinical days. Please make any requests before noon on Fridays.
Do you sell nutritional supplements or medications?
For your convenience, pharmaceutical-grade supplements are available for purchase at our office. We can also help you find inexpensive medications such as non-steroidal anti-inflammatories and other common, non-narcotic medications.
Most medication prescriptions, including those used to treat pain, can be sent directly to your pharmacy when needed. Please be sure to enter your pharmacy information in our medical record.
How Do I Get A Copy Of My Medical Records?
Due to patient privacy laws (HIPAA), we require all requests for records to be submitted in writing. Please either bring a completed records request form with you to your appointment or mail the form to us. A small fee, based on the size of the record, may be charged to prepare your medical records for release unless they are sent directly to another healthcare professional to aid in the continuation of your healthcare. Family members of adult patients must have the patient complete a HIPAA authorization form prior to obtaining records or reviewing personal health information. How Do I Make An Appointment?
To make an appointment, call from Monday through Friday between 8:00 and 5:00. Please bring your insurance or medical cards, photo ID, any special forms required by your insurance carrier and any previous X-rays or imaging studies (MRIs) to your appointment. You may also book an appointment online.
How Do I Pay For Or File My Appointment With Insurance?
For your convenience, payment for office visits may be made by cash, check, or Visa/MasterCard. You may pay via our electronic medical record app called ModMed Pay.
Our office will file your insurance or Medicare forms on your behalf. You will be responsible for any copays or out-of-pocket charges associated with your care at SMOC. This various depending upon your particular insurance plan, deductible and other insurance requirements. You will be asked to pay the copay portion at the time of your office visit. The patient responsibility portion, as noted on your insurance Explanation of Benefits (EOB), will be billed once the insurance portion is paid to the practice.
Some of your treatments may not be covered by insurance but rather paid directly by the patient. These include Orthobiologic treatments and Regenerative Therapies such as shockwave therapy.
